2025 Luminary Awards Nomination Form

Fields marked with a red asterisk (*) are required. To be eligible for this award the nominee must be an employee of a Chamber member in good standing and meet the above criteria. Nominations can be self-nominations or outside nominations. A self-nomination will require an additional letter of recommendation. On completion of this form, the nominee will receive an email prompting them to complete an application for the award. Nominations without the application from the nominee will not have equal weight during the selection process.
